Consent Form for BP Monitor - pending

1. WHAT IS A BLOOD PRESSURE MONITOR?
A blood pressure monitor is a battery operated device that records your blood pressure for 24 hours. It contains an electronic device, a cuff and plastic tubing. The cuff is attached to your non-dominant arm and is connected to the device by tubing. The device will be worn around your waist in pouch.

2. WHY DO I NEED A BP MONITOR?
A blood pressure monitor detects any changes in blood pressure over the time you wear the monitor whilst you go about doing your normal daily activities. The reading that is taken at your GP is similar, but only reads your blood pressure at that single point in time.

Your doctor may recommend for the following reasons:
• High blood pressure
• Fluctuating blood pressure
• To assess if your blood pressure is well managed

3. WHAT ARE THE RISKS OF A BP MONITOR?
There are no risks for this test. Because the cuff must be tight on your arm to make accurate readings, you may experience some pain, numbness, or tingling of your arm or fingers. Bruising or swelling is uncommon.

4. ARE THERE ANY PREPARATIONS BEFORE I DO THE BLOOD PRESSURE MONITOR?
You should preferably have a shower before coming for the procedure. After the blood pressure monitor has been fitted you cannot get it wet. If you need to change your clothes, please ensure the cuff is re-attached tightly to your upper arm.
