What is a coronary calcium score?

A CT calcium score (also known as a coronary artery calcium score) is a CT scan that measures how much calcified plaque is visible in your coronary arteries (the blood vessels that supply your heart muscle). Plaque can build up in the artery walls over time, and some of it contains calcium. The scan detects this calcium and calculates a score.

The result can give your doctor more information when estimating your risk of a future heart attack. It does not measure what percentage of an artery is blocked, and it does not show every type of plaque. Your doctor will consider your score alongside your age, blood pressure, cholesterol, medical history, and other risk factors.

When the scan may be useful

A calcium score is mainly considered for people without symptoms, when the result could help resolve uncertainty about prevention. For example, after an assessment by your GP, a decision about cholesterol medication may be a close call. Evidence of coronary plaque can add to that discussion.

Not everyone with a family history or a raised cholesterol result needs this scan. If treatment is already clearly recommended because of an existing condition or your level of risk, another test may not change the plan. A Heart Health Check with your GP is the usual starting point for assessing your risk.

If you have chest pain

A calcium score is not a way to rule out the cause of chest pain or to check for a possible heart attack. Symptoms need their own medical assessment. Depending on your situation, your doctor may recommend a CT coronary angiogram, a stress test, or urgent hospital care.

What happens during the appointment

  1. Check-in:
The radiographer will check your referral and health details.
  2. Getting ready:
You may be asked to change into a gown and remove any metal items near your chest.
  3. Attaching sensors:
Sticky ECG patches (electrodes) will be placed on your chest so the scanner can time the images to your heartbeat.
  4. The scan:
You will lie on a table that moves through the CT scanner's circular opening. While the images are taken, the radiographer will ask you to keep still and hold your breath briefly. A standard calcium score does not need a contrast injection.
  5. Wrap up:
The images will be checked to make sure they are complete before you leave.

Please note: The scan itself is brief, but changing, positioning, and checking the images all add time to your visit. Scope Radiology will confirm how long to allow for your appointment.

If you've been referred for imaging

Your GP or specialist can determine whether imaging would help and refer you for the right scan. Please have your referral ready when confirming your booking so the Scope Radiology team can verify which scan has been requested and confirm your preparation and appointment details.

A Heartscope cardiologist consultation and a Scope Radiology scan are separate appointments. If you are having both, please confirm where each appointment will take place.

Fees

A CT calcium score is privately billed. Scope Radiology will confirm the amount you will pay before your scan. If you also need a Heartscope consultation, it has its own fee.

Understanding your results

Scope Radiology will prepare a report with your calcium score, which may be compared with people of a similar age and sex, and forward it directly to your referring doctor. Your doctor will explain whether and how the score changes your overall risk assessment, and what it means for prevention in your case.

That conversation may cover cholesterol or blood pressure treatment, smoking, and physical activity. The score does not make those decisions on its own. Do not stop a prescribed medication because your score is low, or start treatment simply because it is high.

A calcium score is not a routine way to check whether a medication is working. Whether to repeat the scan is a separate clinical decision, so do not assume it needs to be done every year.

Preparing for the appointment

This scan is performed by Scope Radiology. Please follow their preparation instructions for your scan.

Frequently Asked Questions

Is calcium scoring done at Heartscope clinics?

No. This scan is performed by Scope Radiology. Your GP or cardiologist can assess whether the result is likely to help and refer you.

Is this a blood calcium test?

No. This scan uses CT images to measure the calcium in coronary artery plaque. A blood test measures the calcium in your blood and answers a different question.

Is a calcium score the same as CTCA?

No. A calcium score is a scan without contrast that measures calcified plaque. A CTCA uses contrast injected into a vein in your arm to show the coronary arteries in more detail, including the channel inside them and plaque that has not calcified.

Does a high score mean I need a stent?

Not necessarily. The score does not show how narrowed an artery is, or whether blood supply is limited. Decisions about further tests or treatment depend on your symptoms and the wider assessment.

Is the test useful if I already have coronary heart disease?

It is usually less useful for decisions about prevention when coronary disease is already established (for example, after a heart attack or a coronary stent). Your doctor can explain whether it would change any decision in your case.

Can a zero score explain away new symptoms?

No. A calcium score is not designed to explain symptoms. Please see your doctor about new chest pain or breathlessness, so it can be assessed in its own right.
